It's most likely the JC Higgins bolt action tube fed shotgun. I have one in 12 gauge though not with the adjustable choke. I am buying another one in darn near mint condition which is a 16 gauge with adjustable choke. My 12 gauge is the 583.17. It should say on the left side of the barrel who made it.
